Dr. Neel Shah

Biography

Dr. Neel Shah is originally from Bangalore, India. He went to Medical School at St. John’s Medical College in Bangalore. Following this, he completed a 3-year Internal Medicine Residency Program at New York Medical College in Westchester, New York. After Residency, Dr. Shah joined Sutter Gould Medical Group in 2010 as a Hospitalist at St. Joseph’s Medical Center in Stockton. In 2011, he moved to Sutter Tracy Hospital where he was one of the two co-founders of the Hospitalist Program there.

Dr. Shah served as Director of the Hospitalist program at Sutter Tracy from 2015-2020, leading the team there to achieve system-wide acclaim with outstanding quality measures, patient outcomes, and patient satisfaction. As a Hospitalist he managed patients in the ICU as well as the Cardiac Telemetry and Medical Units. He has extensive experience in managing and caring for hospitalized patients.

Dr. Shah also has a keen interest in Palliative Care. He completed the CAPC program for Palliative Care. He provided palliative Care Services at Sutter Tracy Hospital and served as Director of Palliative Care there. He also managed Hospice patients who needed hospitalization and end-of-life care. Ever looking to expand his skills and services, in 2020, he joined Dr. Sunil Patel and Dr. Pranjal Patel at PremierCare Physicians. At PremierCare, he continues to manage patients with complex medical problems and chronic conditions at different levels of care- be it Outpatient Office Visits, Hospital Visits, Skilled Nursing Facility, Assisted Living, or Memory Care Units. He provides in-person visits as well as Video/Tele Visits.

Dr. Shah has served in many different leadership roles at Sutter Tracy Community Hospital, serving as Chief of Medical Staff in 2014 and as Chair - Department of Medicine in 2020 and 2021. He has been part of many quality improvement initiatives at Sutter Tracy leading to significant improvement in sepsis care, stroke care, ED admissions, and the Hospital Discharge process.

Dr. Shah is trained in Internal Medicine with a strong focus on Hospital-based medicine, Primary and Preventive Care, Geriatric Medicine, and Palliative Care. He makes regular facility visits at assisted living/memory care facilities in Tracy which is a unique service that no other practice in Tracy offers. He also makes regular rounds at skilled nursing facilities helping manage patients in their transition from hospital to rehab to home.
