Establish Care

Steps for New Patients

Step One
Step 1: Contact Us via Email

To begin the process of establishing care, please email our office at [email protected] In your email, include the following information:

Email Subject: New Patient

Email Body:

  • 1. Photo ID
  • 2. Photo of all insurance cards (front and back)
  • 3. Reason for Appointment: (e.g., Establishing Care, Re-Establishing Care, Urgent Medical Concern – please provide a brief description)
  • 4. Preferred Contact Phone Number
  • 5. Current Home Address
  • 6. Previous Home Address (if applicable)

Step Two
STEP 2: Insurance Verification

We will verify your insurance coverage within 3-7 business days. Once verification is complete, our office will call you at the number you provided to confirm your insurance acceptance and schedule your initial visit.

Step Three

Step 3: Your First Appointment

  • A. You will receive an email with a pdf of your New Patient Packet. We request that youeither:
    • a. Email your completed New Patient Packet to our front office email
    • b. Bring the completed packet to your first appointment or
    • c. Arrive 15 minutes early to fill out your packet
Requirements for Your First Visit
  • Physical copy of your Photo ID
  • Physical copy of All Insurance Cards
  • Completed New Patient Packet
  • All current medication bottles or a photo/list of medications including the medication name, dosage, and frequency

Helpful Information to Bring

  • Immunization Record
  • Last Preventative Screening Dates: labs, Mammogram, PAP smear, Colonoscopy, Diabetic/Glaucoma Eye Exam, Bone Density Report, etc… or other reports you would like your new physician to review.
