Annual Physicals & Wellness Visits

Annual Physicals (<65 Y/O):

We recommend scheduling an annual physical for all patients under 65. During this visit, we’ll review your routine blood work, discuss important screenings, and address any health concerns. Please note that physicals cannot be completed during an initial visit to establish care.

Wellness Visits (65+ Y/O):

For patients 65 and older, we conduct an Annual Wellness Visit, which includes a review of preventative screenings, a memory recall test, and a thorough review of your medical history. These visits are available once a year for all Medicare patients and those over 65. Wellness visits cannot be done during initial appointments to establish care.

Advanced Directive/POLST Form:

An advanced care directive (POLST) outlines your preferences for medical treatment in situations where you cannot communicate. It’s important to update this annually and keep a copy accessible to you or your family

AllCare Insurance Wellness Visits:

For patients with AllCare Insurance, wellness visits are conducted by AllCare-assigned providers, not at our facility. Please contact AllCare to schedule your free Health and Wellness Assessment and inquire about incentives for completing the visit.

Preventive vs. Office Visits

Preventive Visits:

These include annual physicals, wellness exams, and screenings designed to prevent or detect health concerns early. Examples include blood pressure checks, vaccinations, cancer screenings, and overall health assessments.

Office Visits:

These are for addressing specific medical issues or symptoms. If treatment or further action is required during a preventive visit, it may be billed separately as an office visit.
